I am Singaporean, married with a 1 year old child and do not own a property now. My personal monthly alone is above $17k. Are there real cases of approval to buy TOP EC dropout units or EC units close to MOP? Who sends the appeal to HDB and what are the chances?

I would say it will be unlikely given your existing income without mentioning about your spouse. You may wish to look into those that just obtaind MOP for EC as there is no income ceiling to fulfill.

For new EC units purchase direct from developer, yes income ceiling applies, and its based on the household income including your spouse as well. This also applies to pre-TOP and pre-MOP units as well. Income ceiling no longer applies for units post MOP of 5 years, but typically they are selling at much higher price since its semi-privatized by then.

Yes its possible to appeal, and appeal is through HDB. It depends on case by case circumstances review. May I know which new EC you are looking at?
